Print and fill out this form. Mail it with your check or money order (payable to Lithic Designs) and the paper with your baby's footprints (directions below). Delivery will be in four to six weeks. You may pick up the tile to avoid shipping charges.

Directions

Materials: Black Ink Pad, Baby Wipes, White paper

Directions: Apply ink using an inkpad to bottom of baby's foot. Press the foot on the paper using firm, even pressure. Practice a couple of times re-inking between imprints. When you are satisfied with the results of both the left and right feet mail the paper containing the footprints you wish printed on your tile to lithic designs. The footprints do not need to be lined up. Anywhere on the paper is OK.
