Print and fill out this form. Mail it with your check or money order (payable to Lithic Designs) and your photograph. If you use a digital camera, please email the photograph to info@lithicdesigns.com and send your check and the order form by mail. The photograph will be cropped to a square and small defects removed. For a new photograph please do not fill the frame with the subject but leave space at the sides so the photograph can be cropped. Delivery will be in two to four weeks. You may pick up the tile to avoid shipping costs.
